Lamotrigine Dosing in Pregnancy & Postpartum

Let’s begin by answering this question first:

A 28-year-old female with a diagnosis of bipolar disorder is stable on lamotrigine 150 mg daily dose. She recently found out that she is pregnant. After a detailed risk and benefit discussion, she has agreed with a plan of continuation of lamotrigine during pregnancy.

Which of the following lamotrigine dosing strategy will be needed in her pregnancy?

Answer: Gradual increase in dose from first to third trimester; and dose taper within three days of delivery

  • The serum lamotrigine concentration will decrease from the first trimester and reaches its lowest concentration by the mid-third trimester. Both lamotrigine total and free clearance increased throughout pregnancy with a peak of 94% (total) and 89% (free) in the third trimester. This results in a decrease in lamotrigine level.
