Which Antidepressant Is Safest to Start in a Patient on HIV Medications?
Depression is the most common psychiatric comorbidity in HIV — roughly twice the general-population rate — and treating it well protects adherence, and with it, viral control. But the antiretroviral regimen quietly changes how your antidepressant behaves. Here's the fast version of what to start, how to dose it, and what to avoid.
- Start with sertraline or escitalopram. Effective, and relatively clean on most regimens.
- Dose by the regimen bucket. A booster (ritonavir, cobicistat) raises your antidepressant — start low. Efavirenz strips it — titrate up. An integrase anchor (bictegravir, dolutegravir) is clean — dose normally.

Efavirenz induces the very enzymes that clear many antidepressants, so a standard dose can run low. Sertraline can drop by roughly 40–50%; bupropion falls about 55%. Before labeling a patient treatment-resistant and escalating, confirm the dose is adequate for an induced regimen — and remember efavirenz carries its own neuropsychiatric burden (depression, vivid dreams, dizziness) that can look like depression in its own right. In a patient with psychiatric illness, switching off efavirenz is often the cleaner move.
What if my patient also has insomnia, low appetite, or neuropathic pain?
Match the antidepressant to the comorbidity — it's often the cleanest way to choose.
