- Therapeutic drug monitoring (TDM) is valuable for understanding variations in metabolism, adherence, and drug-drug interactions, with a clozapine level between 350 and 600 ng/mL generally considered optimal for efficacy.
- The clozapine-to-norclozapine ratio, typically greater than 1.3 in slower metabolism, can indicate influences like genetic factors, smoking cessation, or medications.
- Elevated norclozapine levels may heighten the risk of sedation and other adverse effects, making monitoring especially relevant for patients who smoke or experience metabolic changes due to illness, age, or inhibitors like fluvoxamine.
